Post-Hospitalization Support

A stay in the hospital can be both a stressful and overwhelming experience. Returning to student life can be challenge. 

Our team is here to support you as you transition back to campus and manage any on-going needs. Our team can support you with:  

• Communicating with professors about missed classes or exams  

• Requesting and understanding your discharge summary  

• Connecting with providers for medical and follow up care coordination

Fill out a Student of Concern form to schedule a meeting with our team to make sure you are connected to the appropriate resources.
